Dear Practitioner,
Your Religious Science Practitioner license as held by United Centers for Spiritual Living is up for renewal this year, IF YOU RENEW IN ODD NUMBERED YEARS.. The renewal period begins November 1, 2011 and ends on December 31, 2011. To be a practitioner in good standing, you must renew your license by December 31, 2011.
Practitioner license renewal fees remain the same this year, $150.00.

IF YOUR LICENSE IS UP FOR RENEWAL AND YOU DO NOT RENEW IT BEFORE
DECEMBER 31, 2011, your renewal fee will be $200.00
($150 + $50 late fee) beginning JANUARY 1, 2012
HOW TO RENEW:
FIRST: Contact your Senior Minister or Head Practitioner
As a renewing practitioner, it is your responsibility to contact your senior minister or the head of your practitioner core and receive their recommendation for you to renew your license and serve in your community. Basic relicensing requirements (CLUs, CEUs, etc.) are outlined in the UCSL Practitioner Code. There may be additional requirements for relicensing as determined by your local Center.
For this conversation, you will want to bring a record of your Practitioner activities. Your center may have provided you a form to use to document your service and education. This is an updated form similar to the one you have used to renew your licenses in the past. If you are renewing online, this form is now only for your own records and for your discussion with your senior minister or lead practitioner and the only place you will have to record your service/education hours. Information on counting service & education hours can be found in Appendix B of the Practitioner Code.
Upon receiving your approval by your minister for renewal, you will be given the new Passcode for the online process.
THIS PASSCODE IS CONFIDENTIAL and for your personal use only. Please honor its confidentiality.
SECOND: Submit Renewal and Fee (either online or hardcopy)
IF YOU CHOOSE TO RENEW ONLINE (PREFERRED METHOD):
***IMPORTANT NOTE----ONLINE RENEWAL FOR $150 AVAILABLE ONLY NOVEMBER 1-DECEMBER 31. CHARGES OUTSIDE THAT PERIOD ARE AT $200***
- Go to http://unitedcenters.religiousscience.org/RenewPrac.aspx
- Most individuals will need to first set up an ID and password first (see note below if you have one already)----Click on “Register” and a “Register Screen” should appear.
- Complete the Registration Form choosing your own personal User ID and password (please keep a record). Where the form says, “Spiritual Name” fill in the name of your spiritual community. Click “Register” at the very bottom of the page and you will receive a message saying that your information has been registered.
- You may now then use your User ID and password to login on the original screen.
- You will then be asked for the “passcode” (this is the code you received from your minister authorizing renewal), enter the code and click on “verify”.
- Fill in your Renewal and Payment Information; your credit/debit card will be charged the amount on the screen ($150 or $200) once you click on “Renew Now.”
- A message will appear asking you to verify the information. When you are satisfied that your information is correct, click on “Renew” and a Confirmation page will appear PRINT OUT YOUR CONFIRMATION and keep it for your records. Be sure to notify Annie Glasgow if you do not get a confirmation email.
NOTE FOR THOSE WITH EXISTING SYSTEM IDS/PASSWORDS---you may skip the registration steps and go directly to signing into system. If you forgot your password, click on the button and follow the prompts to receive an email with your information.
IF YOU CHOOSE TO MAIL IN YOUR RENEWAL:
Download the Courtesy Practitioner Renewal Form from the GEMS webpage (see above), and send the completed form, signed by your senior minister (or Ecclesiastical Representative for those at a church with no minister), along with a check or money order for your renewal fee ($150 before December 31,2011--- $200 if late) made payable to “UCSL” and send to:

Once your payment is processed and your renewal is received, your new ID card will be mailed to you during the month of January, 2012. You should receive it no later than February 15, 2012.
